The Superior Family Health Team is seeking a Physiotherapist who is a dynamic team player to join an innovative, community-based primary health care team.
Working as a Physiotherapist on a multidisciplinary team, you will:
Work collaboratively with other Interdisciplinary Health Professionals to provide comprehensive care plans for our patients
Be responsible for the delivery of patient-centred physiotherapy services
In collaboration with the Primary Care Provider and other members of the interdisciplinary team, develop and discuss appropriate individualized care plan with the patient/family, caregiver based on best practice
Conduct consultations with patient/family to obtain relevant information and assess patient/family learning and needs
Be involved in health promotion, program planning, continuous quality improvement and education for our patients and team members
 Be a resource to Primary Care Providers and other allied health professionals
Your profile will include:
A Bachelors Degree is Physiotherapy
Registered with the College of Physiotherapists of Ontario and in good standing
Membership in the Canadian Physiotherapy Association
Ability to work independently and collaboratively within an interdisciplinary health care team
Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills
